This notice covers the Omni Health Supply website. Provider portal accounts, prescriptions and orders are covered by the agreements and notices that apply to those services.
Do not send patient information here
The forms on this website are for account requests and business inquiries. Do not enter patient names, prescriptions or other health information. Prescriptions are submitted through the provider portal.
What we collect
- Information you submit in a form: name, work email, practice or company, role, phone, state, NPI, interests and message.
- Technical information recorded by our hosting provider when you load a page, such as IP address, browser type, pages requested and time of request.
This website does not use advertising cookies or third-party analytics.
How we use it
- To review and respond to account requests and inquiries.
- To verify provider credentials during account setup.
- To operate, secure and troubleshoot the website.
We do not sell personal information.
Where it is stored and who receives it
Form submissions are stored in a database operated for Omni Health Supply by Supabase, in the United States. The website is hosted by Vercel. Submissions may be shared with companies of the Omni Health Company that fulfill your request, with service providers acting on our behalf, and where required by law.
Retention
Account requests are kept while the request or resulting account is active and as required for business and legal records, then deleted.
Your choices
To access, correct or delete information you submitted, send a request through the contact form with the email address you used.
Children
This website is intended for licensed healthcare providers and businesses and is not directed to children.
Changes
Changes to this notice are posted on this page with a new effective date.
